All OAS forms and waivers (accessible below) are to be retained by the participating school for their records.
OAS makes it easy for schools and coaches to get forms and waivers signed for their archers using the following methods:

Download the forms below to distribute to parents as a fillable PDF email attachment. Completed forms can be sent back to the appropriate school coach or admin.
